Hospital Requests

Hospital discharge

When you are discharged from hospital you should normally receive 7 days’ supply of medication. The hospital will issue you with your new medication requirements. Please bring this to the surgery or post via a stamped addressed envelope before your supply of medication has run out.

Hospital requests for new or amended medication

Hospital requests for change of medication will be checked by the GP if required the doctor will issue you with a prescription. The Practice will endeavour to issue your prescription within 48 hours, but can take longer for more complex requests.

The Doctors will review your medication regularly, which may involve changes to your tablets in accordance with current health Board policies. Please be reassured that this will not affect your treatment.

Private Prescriptions

The practice cannot prescribe on behalf of private providers, nor continue to supply prescriptions which have previously been supplied by the private provider unless this is a medication which a GP would normally prescribe, and the prescription is in line with local health board guidelines. We are not permitted to have shared care protocols with private providers.

Private consultants wishing to transfer patients to NHS care should directly refer their patient to the appropriate NHS service who will reassess and, if appropriate, take over your treatment. At the point that your care has been taken over by an NHS service, a request may be made by the NHS service to share prescribing responsibility with the practice. We will review and manage all such requests in line with the local health board guidelines.

Information:

Urgent Prescription Requests

If you have run out of medication a pharmacy can issue an emergency supply (with a few exceptions) until you can obtain a prescription from the surgery.

Please note we do not issue repeat medication requests on an urgent basis. If you submit a request stating it is urgent this will be reviewed by a doctor and if not deemed urgent for that day the request will go through the normal 48 hour process.

Electronic Prescription Service EPS

A free, secure NHS service.

Instead of printing and signing paper prescriptions, staff at the practice will send them electronically directly to the pharmacy you choose.

What does it mean for me?

It's easier. Your prescription no longer needs to be collected from the surgery and taken to your pharmacy. It will be sent electronically.

It saves time. If you have a phone appointment with the GP or other healthcare professional, you won't have to go to the surgery just to pick up the prescription. Your pharmacy will still need a little time to get your items ready, just as they do now.

It is safe and secure. Your prescription can't get lost, as we can always see on our system where it is.

What do I need to do?

Speak to your pharmacy or a member of staff at the practice and tell them you want to use electronic prescriptions. They will be able to arrange this for you.

You do not need a computer, smartphone or laptop.

The only change is that, in most cases you will not get the green prescription form from the GP practice.
